OFFICE HOURS:  Monday thru Thursday 6:30 AM until 4:00 PM

Appointments

As you might expect, our practice runs strictly on a "By Appointment Only" basis, and we will do our best to accommodate your requested appointment day and time. Our practice offers early morning appointments, giving you the flexibility to have treatment without the need to take time off. If you are unable to attend your appointment, please give us at least 24 hours notice. Please note that there may be a charge for late cancellations or missed appointments.

New patients can download our Medical History Form prior to your appointment.

Emergencies

If you have a dental problem that requires urgent attention, please contact us as early as possible in the day. We will make every effort to see you the same day for any true emergency, particularly if called in prior to 9 AM.

Our practice also offers 24-hour emergency coverage for out of hours emergencies. Please phone the office where the answering machine will provide you with a telephone number for our answering service. If calling to be seen after hours at the office, please note that Dr. Tabone is making himself available in addition to working his normal hours so please make sure that your problem cannot wait. And, please note that a fee will be charged if there is a need for re-opening the office.

Due to medical-legal issues, our after hours emergency service is limited only to registered patients of our practice.

Confidentiality

Patient confidentiality at our practice is taken seriously and all information about our patients is treated with the strictest confidence in accordance with all data protection laws. If you would like a copy of our practice confidentiality policy, please ask our receptionist.

Courtesy Calls

As a courtesy, we attempt to notify you at least a day in advance of any appointments you have scheduled with us. Please note that this is a courtesy only, and should not be relied upon exclusively as a reminder for your appointments.

Insurance

To help defray some of the out-of-pocket cost of your dental treatment, our practice is a participating provider with many dental insurances, including Ameritas, Delta Dental, CSEA, CIGNA, Guardian, and certain Blue Cross plans. We will accept insurance payments in part from other plans to the extent that they will assign benefits, but any patient out-of-pocket expenses are requested to be paid at the time of your visit. We reserve the right to request full payment for our services to you BEFORE your insurance has paid any benefit.

Please remember that you are ultimately responsible for ANY and ALL balances on your account, whether or not you have insurance coverage, or whether your insurance pays a partial benefit, or no benefit at all. Our practice does NOT take Medicaid patients.

Payment Options

For your convenience, our practice accepts payment for services rendered by cash, personal check, Mastercard, Visa, and Discover. We kindly request that payment be made at the time of service, and all balances paid in full at the time of your visit will receive and extra discount.

For large procedures such as dentures or crown and bridge work, full payment must be received BEFORE completion of the treatment.

Accounts that carry a balance that has not been previously arranged with our Financial Coordinator are subject to a monthly interest charge on the account until the balance is paid in full. We employ a collection agency for all accounts in arrears over 45 days.
